Do you qualify?

a. Is your vehicle a 2006, 2007, 2008, 2009 or 2010 model year? Yes No

b. Is the mileage of your vehicle below 160,000 km? Yes No

c. Is your vehicle a car, van, light truck, mini-van or sport utility vehicle which weighs no more than 4,536 kg (10,000 lbs) Gross Vehicle Weight? Yes No

d. Were you the registered owner of the vehicle when the problem arose, do you still own the vehicle?
OR
Were you the Lessee under a lease agreement not less than 12 months when the problem arose, do you still lease the vehicle? Yes No

e. Have you tried to resolve the dispute with the manufacturer (according to the dispute resolution process described in your warranty booklet) and have you given both the authorized dealer and the manufacturer a reasonable amount of time and opportunity to resolve the problem? Yes No
